Works predominantly with male acute psychiatric patients to insure the maintenance of a therapeutic milieu while providing for direct patient care activities including those of daily living, bathing, washing). Maintains through observation and de-escalation training the welfare, safety and security of patients. Works in close collaboration with the treatment team, (i.e.; Physicians, Recreational, Therapists, Dietitian Nurses, Therapists and other Mental Health Technicians), in compliance with JCAHO National Patient Safety Goals and other performance improvement indicators an Continuous Quality Improvement (CQI) activities.
Important areas of responsibility includes:
12. MILIEU MANAGEMENT: Accompanies and transports patients assigned. Demonstrates the ability to identify behaviors that require intervention. Escorts clients when leaving the unit for activities and scheduled tests. Able to perform client safety and unit safety checks at scheduled times, documents appropriately. Responsible for one-on-one observation for high-risk sometimes aggressive patients.. Able to assist with verbal de-escalation and hands on team control, as a last resort in providing safe patient care.
13. TREATMENT PLANNING: Participates in treatment planning, helps identify patient's problems, needs and strengths. Suggests modifications to treatment plan based on observations of the patient's behavior. Provides input into discharge planning. Participates in the implementation of the treatment plan by monitoring patient's progress on daily goals sheet and communicating information regarding patient's behavior with other members of the treatment team.
17. GROUP PROCESS: Facilitates therapeutic groups tailored to the patients' age and developmental level of understanding. Encourages and facilitates patients' participation in therapeutic activities. Supervises patients to ensure their safety and their involvement in treatment; and provides input during shift report. Assists with therapeutic groups under the direction of the Therapist or RN.
19. PATIENT CARE MONITORING: Performs all aspects of client care in an environment that optimizes client safety and reduces the likelihood of medical/health care errors. Monitors patient's compliance with pre and post procedure instructions under the supervision of the RN. Continuously observes clients' behavior, mental status and activities, notifies RN of any changes or unusual occurrences. Provides general care, i.e., transferring clients, assisting with ADLs. Performs and records client vital signs, intake and output; notifies RN of any changes. Orients clients to the unit; takes admission vital signs and records on the Nursing Assessment Form. Participates in client care conferences. Treats clients and families with respect and dignity. Provides emotional support.
