Work Schedule: 8:00am-4:30pm, Monday - Friday Financial Disclosure Report: Not required The primary function of the PT Clinical Assessor is direct patient care, including completing functional assessments to a diverse population in outpatient and community settings within CSP Program of Comprehensive Assistance for Family Caregivers (PCAFC). The incumbent will complete functional assessments of Veteran applicants utilizing a prescribed instrument The Veteran Functional Assessment Instrument (VFAI) and other assessment tools to determine if a Veteran applicant requires personal care services from a caregiver due to inability to perform an activity of daily living (ADL) and/or due to the need for supervision, protection or instruction. In addition to completing the VFAI and other assessments, the incumbent will engage in interprofessional collaboration with social work, nursing, PT, OT, PACT team, MD, PA, DO, NP, psychologists, counselors and a variety of other medical professionals. In addition to completing assessments and interventions within the medical center, the incumbent will complete home visits in the needs within the home. The incumbent will support the staffing needs of the medical center and will complete other duties as assigned from CSP Program management. Evaluation and Treatment: Accept and respond to PT/CSP consults with CSP and Physical Therapy policies. Complete Veteran functional assessments, including the VFAI, for the PCAFC Possess skill in independently utilizing appropriate screening and standardized and non- standardized evaluation tools required to appropriately provide and direct the PT care plan in multiple settings. Responsible for the indirect comprehensive assessment of functional roles and provide direct intervention with varying degrees of complexity to enhance safety, wellness, participation, and performance in all areas of physical functioning of Veterans referred to the CSP. Demonstrates knowledge of and ability to independently apply clinically appropriate assessment techniques and treatment interventions to address and evaluate all aspects of the physical therapy domain: occupations, client factors, performance skills, performance patterns, contexts, and environments. Able to utilize standardized and non-standardized assessments methods to communicate information to the Centralized Eligibility & Appeals Teams (CEAT) and interprofessional CSP Team about Veterans' and caregivers' functioning.
